Maryland residents must hand over valid and acceptable forms of identification, including:

 

  • Military ID
  • Passport
  • Motor Vehicle Administration identification card (front and back) or driving license

The photo must be clear and un-obscured by a hat, head coverings, dark glasses, or sunglasses. It should be taken with a neutral face, perhaps with a smile, facing the camera directly. Besides, it should be clear and colored, taken against a white background, and the patient must be clothed, captured from the head to the shoulders. Technically, the photo should look like a passport or a driving license's photo. Also, the picture should be recent, taken within the last six months, to reflect their current appearance.

 

The photo must also be an image file and shouldn't be digitally manipulated to change the appearance. Besides, patients should keep it below 3MB in size.

 

Note that the MMCC will return your application if it doesn't meet the above requirements.

Hospice care patients must submit a letter from the Attending of Hospice Record (AOHR) doctor indicating that they're in their care. They should also attach a recent photo and submit the attachment in either .pdf or .jpg format.

Please visit the MMCC Registry website, hover over to "Create Account" under the "Patients" heading, and click the button. Then, complete and submit the application.

Also, be sure to verify your email address since the commission won't renew your application without doing that. You can click on the email link sent to you immediately after completing and submitting your application form to verify your email account.

The MMCC will follow up on your renewal with an email informing you whether or not your application is successful. This email usually contains your login details, including your username for the Patient Registry portal. It also lodges your MMCC patient ID number for your doctor to offer your written certification for medical cannabis. You can combine the written certification and Patient ID Number as you purchase medical cannabis in any licensed Dispensary.

If you find having a caregiver viable, you can still go with this optional choice. However, they must submit an application expressing their willingness to commit and get the MMCC's approval.
